A few simple steps to help things run smoothly
To make your first visit easier, we recommend completing your new patient forms before you arrive. This helps us understand your medical and dental history ahead of time so we can focus more on you during your appointment.
Transferring your records
If you are coming from another dental office, you can also sign a records release form. This allows us to request your previous dental information so we have a complete picture of your oral health without extra work for you.
How to complete your forms
You can fill out your forms in whichever way is easiest:
- At home before your appointment
- In the office before your visit
If you plan to complete them in the office, arriving a few minutes early is helpful so we can keep your appointment on schedule.
